  • The Health Information Management Department at Porter Adventist Hospital is committed to offering our patients and families the utmost in care and service in regards to medical record access and retrieval.

    Information on the Health Insurance Portability and Accountability Act (HIPAA) will give you details on your rights. The section on requesting your medical record details how to get copies of your medical chart. Both are included for your convenience and education in Health Information Management. The HIM Department is open Monday thru Friday, 8:00am to 4:30pm, with holiday exceptions.

  • HIPAA was created to address the privacy concerns in a uniform national system for protecting the privacy of an individual’s health information. There are aspects of individual’s health information that are highlighted below for your convenience and information.

    Patient’s Right to Access

    Patients do have a right to access their medical records under the HIPAA Privacy Regulations (Chapter 1, Section 3-2.2) at reasonable times and upon reasonable notice. A patient’s access to mental health records does require additional authorization from the licensed physician who practices Psychiatrist and may be denied if the Psychiatrist in his or her professional scope determines the release of such information to the patient to have a significant negative psychological impact.

    PHI - Protected Heath Information

    PHI means any health information, whether oral, written or recorded in any other form or medium, that: (a) is created or received by a provider, health plan, employer or health care clearinghouse; (b) relates to the past, present or future physical or mental condition of an individual, the provision of health care to an individual, or the past, present or future payment for the provision of health care to an individual; and (c) identifies an individual or with respect to which there is a reasonable basis to believe the information can be used to identify the individual. 45 C.F.R § 160.103.


    The Privacy Regulations provide that an individual has a right to be informed of the privacy practices of their health care providers, as well as to be informed of the privacy rights with respect to their PHI. 45 C.F.R § 164.520(a).

    Record Content and Retention

    Porter Adventist Hospital stores and maintains a complete health information record through means of a hybrid paper record and an electronic component. Together, these two components comprise the entire legal record. All records are kept within the normal course of business.

    Given the federal statutes and state requirements, records must be preserved for no less than 10 years after the most recent patient care episode.

    At Porter Adventist Hospital, we understand that managing your healthcare can be a complicated and confusing process. To help assist with the coordination and management of your healthcare we have provided the Patient Request to Access Medical Records and Patient Authorization to Disclose PHI forms for you to download at your convenience.


    These forms can be used by patients, or their legal representatives, to access and obtain copies of their medical records or authorize the disclosure of the protected health information to third parties.

    Porter Adventist Hospital is required by state and federal law to obtain a complete and properly executed authorization before we can provide access to or copies of health records. All completed forms should be printed and returned to the Health Information Management (Medical Records) department.

    Fees for copies of medical records as pursuant to Colorado State Statue 6CCR1011-1 Chapter 2 part are as follows: 

    $14.00 for the first 10 or fewer pages
    $     .50 per page for pages 11-40 
    $     .33 per page for every additional page over 40

    Credit cards, checks, money orders or exact change accepted.

    NOTE: Fee may be waived if the records are released directly to another health care facility or a physician’s office.
