Information on the Health Insurance Portability and Accountability Act (HIPAA) will give you details on your rights. The section on requesting your medical record details how to get copies of your medical chart. Both are included for your convenience and education in Health Information Management. The HIM Department is open Monday thru Friday, 8:00am to 4:30pm, with holiday exceptions.
