Our rehabilitation program is patient focused...
Achieving the goals of your treatment program will take a lot of practice, effort, and perseverance. We work closely with each patient to determine and attain individual goals, increase self-confidence, and improve overall well-being.
The ultimate goal of each patient’s treatment program is to make the patient a primary contributer to establishing goals and so they can return to as normal lifestyle as possible and become a functional, participating member of their family and community. Throughout your stay, individual goals will be set and monitored by you and your medical team. The rehabilitation unit uses an interdisciplinary team approach, making the patient and family members an integral part of the team which consists of your personal doctor, the unit medical director, nurses, occupational therapists, physical therapists, speech therapists, and case managers. Your doctor will be provided up-to-date reports on your progress.
Program Components...
Conferences
During the first week of admission, the rehabilitation team will hold a conference to analyze your condition and progress. Proposed goals and estimates of the necessary time frame will be established. Throughout treatment, the team will meet regularly to evaluate treatment goals, report on progress and modify your program as necessary. Conference results will be shared with you and your family.
Therapeutic training
Each day includes a number of therapy sessions, structured to meet your goals, which may include physical, occupational, speech therapy and/or other therapies, as needed. In order to make the most effective use of your therapy and treatment and further progress we encourage you to be on time for all sessions.
Family involvement
We strongly support the vital role of family members and friends in your rehabilitation process. We encourage and welcome them to observe therapy sessions or nursing care activities at your request. Please make arrangements with the primary nurse as it may be necessary to observe therapy outside regular visitation hours. Family teaching sessions are also planned to provide special training for family members who need to assist the patient at home.
Clothing and personal needs
You will soon be doing daily self-care activities on your own including dressing, bathing, getting in and out of bed and walking to and from the dining room. Comfortable, everyday clothing is best and should be labeled appropriately. Please bring any personal items such as toothbrush, hair care items or make up that you will need. For your security, please leave any valuables and jewelry at home.
Discharge planning
The case manager will work directly with you and family members to plan your discharge day. If you will not be returning to your previous living location, the social worker can provide you with information and assistance in selecting a suitable alternative.
